Faith can provide a service or a combination of services in your home – all under the direction of a physician.  Working with your doctor, our qualified interdisciplinary team will plan, coordinate and provide care tailored to your needs.  Our team includes:

• Skilled Nursing Services:
  • Provided by an RN or LPN with training and experience in providing care in the home.  Nurses communicate frequently with your physician to update your plan of care.  Services may include wound care, administration of IV medications, teaching, cardio pulmonary monitoring, etc.
• Physical, Occupational and Speech Therapy Services:
  • Provided by licensed therapists or licensed therapy assistants under the direction of the therapist.  Your therapist will provide specific information about the services and treatments you will receive.  Services may include strength and balance exercises, gait training, ultrasound, activities of daily living, teaching, swallow techniques, etc.
• Medical Social Work Services:
  • Provided by licenses medical social workers.  Services may include short-term counseling services, referrals to coordinate with community resources and assistance with living arrangements, finances and long-range planning.
• Home Health Aide Services:
  • Services are provided under the supervision of a registered nurse or licensed therapist.  Our aides have experience and training in providing care in the home.  An aide is assigned when there is a specific need for personal care on a part-time basis in the home.  Typical duties include bathing, shampooing hair, changing bed linens, and assistance with other activities of daily living.
• Dietician: 
  • Services to assist with special diets and education.
• Supplies:
  • All medically necessary and reasonable disposal medical supplies will be provided while receiving services.
