Bereavement services are an integral part of our Hospice program. Following a life limiting diagnosis; patients and families often struggle with grief and other issues. The Medical Social Worker and Chaplain are available to assist families during this emotionally difficult time. Following the death of a patient, Faith’s Hospice program continues to provide support to grieving families and caregivers. While one individual is identified as the primary contact with the family, generally the spouse or primary caregiver, all members of the family are eligible to receive bereavement support. These services are available for a period of 18 months following the patient’s death. Services:
