Admission to our Hospice Program is made upon recommendation by your physician, based upon your needs.  Normally, appropriate candidates for hospice are individuals who:

  • have a life expectancy of 6 months or less
  • desire symptom management instead of aggressive treatment for their illness
  • want to stay in the home of their choice; with the support of family, friends or paid caregivers

On admission, our nurse will visit you and/or your family to discuss Hospice services, assess your immediate needs and recommend a plan of care.

Some of the common conditions appropriate for hospice services are:  Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), End- Stage Dementia, Cancer, Renal Failure, Stroke, Coma, ALS and other neurological conditions.
